No. North Carolina is a direct access state, which means you do not need a referral from a physician in order to receive a physical therapy evaluation or treatment. You can schedule directly with Dr. Ali.
Adapt Wellness is a cash-based practice. The current insurance model for physical therapy doesn't support the depth and continuity of care we provide. Our private pay model allows Dr. Ali to work toward your goals without being limited by insurance visit caps or protocols.
You may be eligible for reimbursement through your out-of-network benefits. We're happy to provide a superbill — an itemized document with all the information needed to submit to your insurance company. We also accept HSA and FSA cards.

In the comfort of your own home. Dr. Ali comes to you — arriving 10–15 minutes before your appointment to set up a treatment table, sheets, and all necessary equipment wherever you're most comfortable. Treatment is provided within a 10-mile radius of Uptown Charlotte, with a travel fee for the 10–20 mile range.
Your first session is a thorough 60-minute evaluation. Dr. Ali will review your history, discuss your symptoms and goals, and conduct a physical assessment. For pelvic floor concerns, an internal examination may be recommended — but it is always optional and only performed with your full consent. You may bring a friend or family member as a chaperone. Dr. Ali will explain every step before it happens.
Pelvic health conditions including pelvic pain, pain with intercourse, urinary incontinence, prolapse, constipation, bowel dysfunction, painful bladder, diastasis recti, endometriosis, PCOS, sexual health, postpartum care, and gender-affirming surgery rehabilitation.
Orthopedic conditions including neck and back pain, shoulder injuries, hip pain, knee conditions, and ankle instability. Dr. Ali is also a professional yoga therapist and can support your return to yoga practice after injury.

We require 24 hours notice for any cancellations or changes. Cancellations with less than 24 hours notice will be charged 50% of the visit cost. No-shows will be charged the full visit amount. Your appointment time is reserved specifically for you — this policy allows us to offer that time to other patients who need care.
